Lance C. Balk Director
Lance C. Balk has served as a director since 2010. He brings significant legal and healthcare business experience, having most recently served as senior vice president and general counsel for Siemens Healthcare Diagnostics. Prior to his position at Siemens Healthcare Diagnostics, Mr. Balk served as senior vice president and general counsel for its predecessor company, Dade Behring. Mr. Balk was part of the Executive Leadership Team of both Siemens DX and Dade Behring. In addition to handling all global legal matters for both companies, during his tenure at Dade Behring, Mr. Balk also was responsible for all public company and board of directors’ matters. Prior to joining Dade Behring, Mr. Balk was a Partner at Kirkland & Ellis where he co-founded the firm’s New York corporate and securities practices. Mr. Balk also serves as a member of the board of directors of Belden Inc., a NYSE listed manufacturer of cables and connectivity products. He received a bachelor’s degree from Northwestern University and a J.D. and M.B.A. from the University of Chicago.
Sam R. Leno Director
Sam R. Leno has served as a director since 2006. He has served as the Chief Operations Officer since March 1, 2010 of Boston Scientific and prior to that, from June 2007 until March 2010, he served as Chief Financial Officer and Executive Vice President of Finance and Information Systems Corporation of Boston Scientific. From 2001 to May 2007, Mr. Leno has served as the Executive Vice President, Finance and Corporate Services and Chief Financial Officer for Zimmer Holdings, Inc. From March 1999 to 2001, he was Senior Vice President and Chief Financial Officer of Arrow Electronics, Inc., a global distributor of electronic components. Mr. Leno served from 1971 to 1994 in financial management roles at Baxter International and its predecessor, American Hospital Supply, ultimately serving as Vice President Finance and Information Technology Hospital Business. Mr. Leno holds a BS in Accounting from Northern Illinois University and an MBA from Roosevelt University.
Thomas Rockwell Mackie, PhD Chairman of the Board
Thomas Rockwell Mackie, PhD co-founded our company in 1997 and has served as Chairman of our Board of Directors since 1999. Dr. Mackie also served as President of TomoTherapy Inc. from 1997 until 1999 and as Treasurer from 1997 until 2000. Since 1987, Dr. Mackie has been a professor in the departments of Medical Physics and Human Oncology at the University of Wisconsin, where he established the TomoTherapy research program. Dr. Mackie also co-founded Geometrics Corporation (now merged with ADAC Corp.), which developed a radiotherapy treatment planning system. Dr. Mackie currently serves as President of the Medical Physics Foundation, and as a member on the boards of Cellectar Inc., a drug development company, Bioionix Inc., a water treatment company, and the University of Wisconsin-Madison Calibration Laboratory. Dr. Mackie has a BSc in Physics from the University of Saskatchewan and a PhD in Physics from the University of Alberta in Edmonton.
H. Jonathan McCloskey Director
Jonathan McCloskey has served as a director since 2009. He has served as Vice President of Investments of Parkwood Corporation, an investment company that invests in private and public companies since March 2010. Prior to that, from December 2003 to March 2010, Mr. McCloskey managed the interests of Avalon Capital Group, Inc. in its private and public portfolio companies. From 1996 to 2003, he served as a Vice President of Investment Banking for First Albany Corporation in New York. Mr. McCloskey is a Director of the San Diego County Board of Retirement, which oversees the investments of the county’s retirement fund. Mr. McCloskey received a BS in Economics from Pennsylvania State University and an MBA from Yale University.
John J. McDonough Director
John J. McDonough has served as director since 2004. He co-founded and in 2001 was appointed Chairman of the board of directors of McDonough Medical Products Corporation, one of the largest independent suppliers of x-ray tubes, medical imaging components and dental x-ray systems in the United States. Prior to that, Mr. McDonough served as Vice Chairman of DENTSPLY, a manufacturer of artificial teeth and consumable dental products, in 1995. Mr. McDonough also served as Vice Chairman and Chief Executive Officer of DENTSPLY from 1993 to 1995, and as Chairman and Chief Executive Officer of GENDEX Corporation, a manufacturer and distributor of dental x-ray equipment, from 1983 until 1993 when GENDEX merged with DENTSPLY. Mr. McDonough has a BS in Accounting from the University of Notre Dame and is a CPA.
Cary J. Nolan Director
Cary J. Nolan has served as a director of TomoTherapy since 2001. He served as President and Chief Executive Officer of Picker International from 1989 to 1999. Prior to that, from 1987 to 1989 Mr. Nolan served as President and CEO of Cooper Surgical, a division of Cooper Companies, and from 1978 to 1987 as President of Xerox Medical, a division of Xerox Corp. Mr. Nolan also serves as a director of Premier Farnell plc and of Toshiba Medical Corporation. Mr. Nolan has a BA and a BSEE from the University of Notre Dame.
Carlos A. Perez, MD Director
Carlos A. Perez, MD has served as a director since 2005. He is a former President of the American Society of Therapeutic Radiation and has more than 40 years of experience in radiation oncology. Dr. Perez is Professor Emeritus in the Department of Radiation Oncology at Mallinckrodt Institute of Radiology, Siteman Cancer Center, Washington University Medical Center, having joined the staff in 1964. He received his training in Radiology and Radiation Oncology at Mallinckrodt Institute of Radiology, Washington University Medical School and was a Fellow in the Department of Radiation Therapy at M. D. Anderson Cancer Center in Houston, Texas. Dr. Perez received a BS and his MD from Universidad de Antioquia, Medellín, Colombia, S.A.
Frederick A. Robertson, MD Director and CEO of TomoTherapy Inc.
Frederick A. Robertson, MD has served as our Chief Executive Officer and a director since joining the company in January 2005. Prior to joining TomoTherapy, Dr. Robertson served as an Assistant Professor of Anesthesiology at the Medical College of Wisconsin from 2000 through 2004. From 1998 to 2000, Dr. Robertson served as President and Chief Executive Officer of GE Marquette Medical Systems, and later as Chief Clinical Officer of GE Medical Systems. Dr. Robertson previously held management positions with Marquette Medical Systems, including President and Chief Executive Officer, President-Patient Monitoring Division and Medical Director. Dr. Robertson also serves as a director of the University of Wisconsin Foundation and Access Genetics, LLC, a molecular diagnostics and interpretation company. Dr. Robertson has an MBA from San Diego State University and an MD from the University of Wisconsin Medical School.
Roy T. Tanaka Director
Roy Tanaka has served as director of TomoTherapy since 2008. Mr. Tanaka brings to the TomoTherapy board 27 years of operational leadership experience, including global market expansion. In 2008, Mr. Tanaka retired from Biosense Webster, Inc., a Johnson & Johnson company, where he most recently served as Worldwide President. Mr. Tanaka joined Johnson & Johnson in 1997 as the U.S. President of Biosense Webster, Inc. before being named to the position of Worldwide President. From 1992 to 1997, he served in a variety of senior management positions at Sorin Biomedical Inc., including President and Chief Executive Officer. From 1989 to 1992, he served in vice president roles with Shiley, a division of Pfizer Incorporated. Mr. Tanaka serves on the boards of Volcano Corporation and VytronUS, Inc. Mr. Tanaka received a Bachelor of Science degree in Mechanical Engineering from Purdue University and an MBA from Illinois Benedictine College.
Frances S. Taylor Director
Frances Taylor has served as a director since 2006. She has more than 28 years experience in corporate and investment banking both within the United States and internationally. From December 2005 to July 2007, Ms. Taylor served as Interim Director and General Manager of Builders World, Inc., a distributor of building materials. From 1977 until 1998, she served as an Executive Vice President at Bank of America and from 1970 to 1977 as a Second Vice President at Smith Barney. Ms. Taylor serves on the boards of the University of Wisconsin Foundation, UW-Madison International Studies Advisory Board, Oak Financial, Oak Bank, Wisconsin Chamber Orchestra and Madison Rotary Foundation. Ms. Taylor holds a B.S. in Education from the University of Wisconsin.
